  • Scope tested What was in scope for testing, and what was left out.
  • Test results Planned, executed, passed, failed and blocked scenarios.
  • Open defects and severity Defects still open, with severity where known.
  • Risk accepted by the business Anything the business agreed to go live with, and who signed it off.
  • Acceptance criteria met or not How many criteria passed, and which did not.
  • Cutover, rollback and support Is the cutover planned and rehearsed, can you roll back, and are support and users ready?
  • Go-live date and dependencies The planned date and anything it depends on.
  • Where the facts came from Source and date of the main figures, so stale ones can be flagged.
